Photo/Video Permission and Release Form

I hereby give the University of Bridgeport the absolute right and permission to record my likeness and/or voice on video, audio, digital, electronic, photographic or online format, and to use, reproduce, publish, exhibit, edit or distribute, in whole or in part, these recordings in all media and formats without compensation for any purpose that the University deems appropriate. I further authorize the University of Bridgeport to use my name in connection with these recordings. I hereby release the University of Bridgeport and those acting pursuant to its authority from liability, claims, and demands for violation of any personal or proprietary right I may have in connection with such use.
